Transitional Housing Program (THP)
- Senior Community Centers’ Transitional Housing Program has successfully operated for nine years.
- THP has 35 units at the Sara Francis SRO Hotel and two assisted living units at St. Paul’s Senior Homes
- SCC case manages 20 of the units; the remaining 15 are case managed by the San Diego Police Department Homeless Outreach Team (HOT), Downtown Impact and Traveler’s Aid.
- 107,350 total bed nights provided since 2002
- 921 seniors provided emergency housing since 2002

- Senior Community Center (SCC) staff has screened 1,534 unduplicated homeless seniors.
- 60,819 total bed nights provided
- 478 total number of clients placed in program
- 359 total number of clients who moved into more permanent housing
- 90% success rate assisting seniors in finding more permanent stable housing
- Average length of stay is 127 days
THP Details
Program goal is to assist clients in finding affordable stable housing; thus, clients are required to meet with a case worker a minimum of 1 meeting per week
The case management includes removal of barriers that contributed to client’s situation (e.g., no ID, unemployed, being terminated from entitlement programs for no address, evictions on record, poor credit, mental & physical health issues, awaiting SSI, applying for SSA).
Effective February 13th, 2012: Walk-in assessments for the THP are available Tuesday afternoons from 1:00pm to 3:30pm at the Gary & Mary West Senior Wellness Center located downtown at 1525 4th Avenue, San Diego, CA 92101
How do we fund the THP?
- The City of San Diego pays for the rent on the rooms
- County of San Diego pays for the case management
