Malcolm - After suffering a series of health problems, Malcolm was left unable to work and unable to shop or cook for himself. With no family able to support him, he faced the possibility of losing his independence, or worse, of living alone on the streets.
Malcolm began eating two meals-a-day at our Broadway Day Center where he also received case management, counseling, and referrals to services. His case manager referred him to our affordable, supportive housing project, Potiker Family Senior Residence. Shortly after moving in, the illness hospitalized him and he had to have both legs amputated. He faced a lengthy recovery and the possibility of being placed in a skilled nursing facility.
The supportive services team at Potiker rallied around him and made it possible for him to remain living independently by mapping out a treatment plan with the nurse case manager, enrolling him in our Home Delivered Meals program, assisting him in locating in-home services, and keeping a watchful eye on him until he was strong enough to get along on his own.
Two years later, Malcolm still lives at Potiker and is one of its most active residents. He continues to utilize the supportive services available and especially enjoys the variety of ways to get involved.
