Health Benefits Advocacy

Three full-time Consumer Center bilingual advocates work Monday – Friday, with offices in the Gary and Mary West Senior Wellness Center. These advocates provide seniors with free legal assistance regarding Medicare, Medi-Cal, and other health care access issues. Passport to Health Program

The Passport to Health Program was implemented in collaboration with the West Health Institute and the West Foundation to empower seniors to take control of their health through services provided at the Gary and Mary West Senior Wellness Center. The Program is a unified effort between social workers, a Nurse Case Manager and a Geriatric Care Coordinator.

The Program utilizes technologies and tools that foster client engagement, self-management and independence, while the Passport team provides guidance and support, making recommendations for follow up strategies and encouraging participation in socialization opportunities. The primary goal of the Passport to Health Program is to promote and support senior wellness through an innovative coordination strategy, delaying and/or preventing more intensive care, emergency room visits or a move into a care facility.

Mental Health Services

We provide culturally sensitive and supportive on-site mental health education, counseling, referrals and assessments. Through a partnership with Sharp Mesa Vista Hospital, psychiatric nurses and a psychiatrist are helping seniors live a more stable life. For more information, please call 619-487-0719.
